What type of BBS?  I know that WWIV will capture the screen/keyboard
interrupts and automatically reroute them to/from the modem.  In other
words, when I was writing my door ("Swords of Reunion") for WWIV, I just
wrote it in C++, and let the BBS program handle the modem I/O...
