It doesn't have everything you're looking for but it might be a good 
place to start...

Check out my timesheet program in the archives.  You can set up 
projects, make notes about what you did each day for the project and 
track time spent on the project.  You can even print a timesheet.  It 
doesn't have the ability to set priorities but you could probably add it 
without a lot of trouble.
