The current indexing follows the "traditional" layout:

Reasoning:
- The current index has no page breaks (it is a long scroll down the page)
- i.e. have to scroll 3 pages to view the A's while looking at the unwanted M's

I have found a way to make an index for the 4.0 documentation.
Reduce this to a "suggestion only" for 4.1 .
